SD Card name changes everytime I restart

I have a SanDisk 256 card that I got when I bought 400 mhz Axim. Everytime time I switch on then off the PDA (clicking the top middle button), the SD card is named as either "Sd Card" or "SD Card2" alternately; and everytime I am asked if I want to format it or not. Ofcourse I select "No". This card was formatted the first time and then data was stored on it.

Questions:
1) Why does OS recognize the card by names mentioned earlier when PDA is switched on and off?
1A) Why am I given an option to format SD Card everytime I either insert it or switch on/off PDA

2) When I soft reset my PDA all the data that was stored on this PDA was lost!! Any reason why this might have happened?

Sounds to me like the card is bad.

I have had similar problems with Sandisk 128 SD card, and affter looking around, it seems like Sandisk and Axim don't get along too well.
I am in the process of contacting Sandisk about getting a new one, but not much luck yet.
