fatal alert

Hi guys, new member here, and somewhat technically inept!!!
I got a new m130 a few weeks ago and it came with a palm productivity card and when I try to use the quickID on the card, it comes up with a fatal error screen "form.c,line5094,no form to return to"
what the heck does this mean??, should I do a soft reset?

btw, when I hit the reset button, it goes into the palm powered screen for a second or 2 then comes up at the date and time preferences screen......
