iPaq Cannot Format SD

I really need help here...
I've been using a Transcend 128MB SD card for the past 2 weeks (used it straight from the box...)

Suddenly, my iPaq 3970 stop reading it and every time I insert the SD, it pop a message "Storage Card Warning"... asking me to format the card. If I choose NO, the card become 0 byte. If I choose YES, it pop a message "An error occurred while formatting the Storage Card folder (error code 0)"

I though the SD is gone, but when I use my USB card reader, it is OK (the contents is still there)... so I format it using the USB card reader, but still get the same message... I been trying formatting it with all kind of FAT but still no success...

Anyone can help me please...

This may be obvious, but have to ask. Have you done a soft reset on the Pocket PC? I've had this issue in the past with a couple different model pocket pc's and hand held pc's. A soft reset has helped me when it lost the card like that.

I actually reinstall the ROM 2.1 again, I ran out of ideas, so I thought by reinstalling the ROM (like fdisk I guess) the problem will be fixed, but no...

I guess that is more than soft or hard reset...
